- Data Entry:
Treatment – Select Chelation, Wound Excision, or Wound Irrigation from the dropdown list.
Chelating Agent – Select the specific chelating agent that was administered to the registrant (chelation treatments only).
Modality – Select the administration mode (e.g. oral or i.v.) for chelation treatments.
Date/Time – Enter treatment date and time.
Dosage – Enter the dosage for chelation therapy as it appears in the hard file (do not perform unit conversions).
Dosage Unit – Select the unit used to report the dosage in the hard file.
Activity Removed – Enter the activity removed by excision as it appears in the hard file (do not perform unit conversions).
Activity Unit – Select the unit used to report the activity removed in the hard file.
Comments – Enter any additional treatment information here.
Source – Enter the source of treatment data as: document date – document name (e.g. 5/22/79 - internal dosimetry memorandum from site health physicist). Do not include specific personnel or registrant names.
