Treatment – Select the type of treatment that was administered from the dropdown list.
Modality – Select the administration mode (i.v. or oral) for chelation treatments.
Date/Time – Enter treatment date and time.
Dosage – Enter the dosage for chelation therapy as it appears in the hard file (do not perform unit conversions).
Dosage Unit – Select the unit used to report the dosage in the hard file.
Activity Removed – Enter the activity removed by excision as it appears in the hard file (do not perform unit conversions).
Activity Unit – Select the unit used to report the activity removed in the hard file.
Comments – Enter any additional treatment information here.
Source – Enter the source of treatment data as: document date – document name (i.e. 5/22/79 - internal dosimetry memorandum from site health physicist). Do not include specific personnel or registrant names.
