- Data Entry:
Contaminant(s) – Select the contaminant (radionuclide or type of radiation) or mixture from the drop down list.
Time Pulled – Enter the date and time that the air sample was removed from the air sampler;
Location – Enter the area, building, and/or location within the building that the air sample was located.
Sample Period – Enter the air sampling period.
Sample Period Unit – Select the unit used to report the sample period.
AMAD – Enter the activity mean aerodynamic diameter (AMAD) in µm, if known.
Reported Concentration Value – Enter the activity concentration as it appears in the hard file (do not perform unit conversions).
Reported Concentration Uncert – Enter the uncertainty associated with the concentration as it appears in the hard file (do not perform unit conversions).
Reported Concentration Unit – Select the unit used to report the concentration and uncertainty in the hard file.
Comments – Enter any additional pertinent information here.
Source – Enter the source of incident data as: document date – document name (e.g. 5/22/79 - internal dosimetry memorandum from site health physicist). Do not include specific personnel or registrant names.
